PRIVACY & SECURITY
We protect your information using physical, technical, and administrative security measures to reduce the risk of loss, misuse, unauthorized access, disclosure and alteration. We use PCI (payment card industry) and HIPAA (health insurance portability and accountability act) security standards. Some of the safeguards we use are firewalls and data encryption, physical access controls, and information access controls. We make every reasonable effort to protect our customers' private information and to store it securely in accordance with the above standards.
Clients may be required to place a credit card on file or prepay to reserve a session. InHabit Wellness, LLC will charge a card for agreed upon services and in accordance with these policies. Card information will be stored for future transactions. A client may cancel this authorization at any time by notifying our office in writing. This authorization will remain in effect until cancelled.
Payment is due when services are rendered.
We accept all major credit cards, Venmo, Applepay, PayPal, HSA and FSA cards.
There is a $35 charge for all returned transactions.
All sales are final. No returns or exchanges.
Any prepaid session(s) remain on the clients account unless the client requests to transfer the account credit to another individual. Prepaid sessions expire one year from date of purchase.
Clients are financially responsible for their reserved session time. InHabit Wellness, LLC will charge the full session fee under the following conditions:
Less than 24 hour notification to cancel or reschedule your appointment.
No show or missed appointment.
Late arrivals- arriving late for a service may require shortening the length of the session.
Clients are not allowed to enter our office or receive bodywork services if they or anyone they’ve been in contact with has had a temperature at or above 100F in the past 72 hours or experiencing any of the following COVID-19 symptoms:
Fever, cough, shortness of breath or difficulty breathing, chills, repeated shaking with chills, muscle pain, headache, sore throat, new loss of taste or smell, nausea or vomiting, congestion, runny nose.
Clients fully acknowledge that a person can unintentionally spread COVID-19 to others even if they do not feel sick or have symptoms.
During the pandemic, clients who choose to receive bodywork services from InHabit Wellness, LLC:
Must not have been diagnosed with COVID-19 in the past 60 days.
Knowingly been exposed to anyone with COVID-19 within the past 30 days.
Have not traveled to any areas greatly impacted by COVID-19 (Hot Spots) or out of the country in the past 30 days.
Have not experienced any of the above symptoms within the last 72 hours.
Agree to wear a face covering when entering our office and in all common areas. Face coverings are not required while receiving treatment.
Agree to wash hands before & after the session.
This agreement and the requirements therein apply to all visits at InHabit Wellness, LLC. If at any time before a scheduled visit health status changes, clients must notify our office immediately and reschedule the appointment until they are cleared to return.
Clients acknowledge that InHabit Wellness, LLC cannot completely control the spread of COVID-19 and have chosen to enter this business and consent to receive close contact service with full knowledge of the risk of contracting COVID-19 where social distancing cannot be observed. InHabit Wellness, LLC will not be held liable for any risk or exposure to COVID-19 a client may receive while in our office.
CBD, TOPICALS & AROMATHERAPY
A client may request sessions include the use of Cannabidiol (CBD), topical analgesic infused creams, salves, ointments, oils and/or aromatherapy. If such request is made, the client consents to the use of CBD and other topicals in all subsequent sessions. While no research has been conducted that indicates any risk, InHabit Wellness, LLC will not use CBD-infused topicals on pregnant or lactating women.

Clients receiving cupping therapy agree to have read over the cupping therapy guidelines and follow the instructions to the best of their ability. Clients who choose to experience cupping therapy understand the potential side effects, risks and benefits. Any markings, discoloration, and redness to the skin will dissipate from a few hours to as long as two weeks in relation to after-care activities and individual healing abilities. InHabit Wellness, LLC will not be held liable for any injury sustained from the use of cupping therapy equipment, regardless of compliance to these guidelines.
We provide bodywork services for stress reduction, relaxation, relief from muscular pain and tension, and improvement of circulation and energy flow.
Therapeutic techniques performed may include but not limited to: Deep Tissue, Neuromuscular Therapy, Cupping Therapy, Swedish Massage, Sports Massage, Active Isolated Stretching, Trigger Point Therapy, Graston Technique, Pre-Natal, Myofascial Release. Use of Ultrasound, Electric Stimulation, Fascia tools, Theragun and other soft tissue tools may be used during the session.
The services offered are not a substitute for medical care. InHabit Wellness, LLC is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness.
If a client experiences pain or discomfort during the session, they are to immediately inform the therapist so that pressure/strokes can be adjusted to their level of comfort. Clients agree to not hold InHabit Wellness, LLC or therapist responsible for any pain or discomfort experienced during or after the session.
If uncomfortable for any reason, the client or therapist may cease the service and the therapist will end the massage session.
Client agrees to notify InHabit Wellness, LLC of all known medical conditions and injuries and inform our office of any changes in health and/or medical condition. There shall be no liability on InHabit Wellness, LLC should a client fail to do so.
The bodywork services provided are entirely therapeutic and non-sexual in nature. Our office has a zero tolerance policy for any sexual behavior.
Client agree to abide by the terms, conditions and policies noted above. InHabit Wellness, LLC reserves the right to modify/change rules, regulations, services and pricing with reasonable notice.
By receiving services from InHabit Wellness, LLC, clients agree to waive and release InHabit Wellness, LLC from any and all present and future liability, loss, cost, claim, or damage whatsoever, which may be imposed upon InHabit Wellness, LLC. Clients further undertake to indemnify and hold InHabit Wellness, LLC harmless from any incident(s) arising from the use of our services.
This membership agreement is entered into on the date of purchase by and between InHabit Wellness, LLC (“We” or “Us”) and Member (“You”) and will continue on a month to month basis as of the date of purchase until 30 days following a written cancellation is received.
Member agrees to be bound by the terms of this Agreement and the Terms and Conditions of Membership which have been provided in this format to the Member.
Member agrees to notify InHabit Wellness, LLC about any changes in address, contact information and/or credit card information.
By purchasing a membership either in person, by phone or online, Member authorizes InHabit Wellness, LLC (or authorized representative) to charge the credit/debit card used at the time of purchase for all charges incurred by the Member including recurring monthly membership dues and other ancillary charges incurred by the Member in association with their membership.
Membership payment will be deducted on the start date and every 30 days thereafter. Services may be redeemed after payment is received.
Memberships are non-transferable and non-refundable. Membership credits are subject to our appointment cancellation policy.
Terms & Conditions
Memberships are for a minimum term of 90 days. Following the initial 90-day term, member understands and acknowledges that the original membership plan purchased will automatically renew on a month-to-month basis until the membership is cancelled.
Member has elected to pay for the membership plan purchased. Membership fee covers the services and benefits provided within that specific membership plan.
Age: We do not perform bodywork services on minors. You must be age 17 years or over to become a member and/or participate in bodywork services at InHabit Wellness, LLC.
Each Member receives the bodywork service credits provided in the plan purchased.
Heal Plan = 4 bodywork sessions per month
Transform Plan = 2 bodywork sessions per month
Support Plan= 1 bodywork session per month

Transfer & Shared Credits
A member can transfer an existing credit to any non-member.
A limit of 2 member-priced service credits for each individual non-member in a 6-month period. Non-members using member service credit will not receive any other member benefit.
Two members can share member benefits between them. Example: If one member has 2 credits they cannot use but the other member has fully redeemed their credits, they can transfer their credits to the other member to redeem.
Members may rollover any unused services to the following month up to 90 days before they expire. Unused credits are non-refundable/returnable after 90 days. Upon cancellation of your month to month membership, accumulated member service credits expire and are non-refundable 90 days after you cancel your membership. Accumulated member service credits expire 90 days after the last day of the prepaid term.
Unless provided otherwise by applicable law, you may cancel your membership only if:
You permanently relocate your residence more than 20 miles away from InHabit Wellness, LLC.
A physician certifies that you are unable to receive bodywork services.
An active licensed massage therapist with InHabit Wellness, LLC determines that you are no longer able to receive services.
Once the membership has been effectively billed for the initial 90-day term, the member may cancel membership for any reason with a 30-day written notice, or such other notice required by law.
All cancellation requests must be accompanied by written proof, signed, dated, and submitted by email, hand-delivered to InHabit Wellness, LLC office, or certified postal mail.
All cancellation requests will become effective 30 days from the date the cancellation request is received and processed by InHabit Wellness, LLC. Payments due prior to the effective date will be charged as scheduled and member will have 90 days to use the credits associated with membership after the final charge
Membership Freeze Option
A membership may be frozen for a minimum of one month and a maximum of three months.
Memberships can only be frozen two times in a 12-month period for a maximum of 3 months at a time. A membership must be unfrozen a minimum of 30 days before it can be frozen again. A "freeze fee" may apply.
Members can redeem accumulated services during a freeze but cannot utilize any additional membership benefits (i.e. discounted gift card purchases, discounts on services and products).
The minimum 90-day initial term must have been completed before a membership is eligible to be frozen or cancelled.
Freezing a membership is completely at the discretion of InHabit Wellness based on the individual member circumstances.
